Self-Pay Program FAQs
How do I qualify for the self-pay program?
You can qualify for the self-pay program if you do not have any health insurance. Health insurance includes but not limited to private health insurance, Medicare, Medicaid, employer-sponsored health insurance, settlements from a liability insurer, Worker’s compensation, No-fault auto insurance, and long-term care insurance.
How much do I have to pay for a consultation?

I want to come in for a general office visit. What is included in the fee for a general office visit?
The $300 fee for a general office visit includes the healthcare provider’s consultation, physical examination, and evaluation. This also includes in-house urinalysis, rapid strep test, rapid flu test, urine pregnancy test, wet mount (microscopic observation of vaginal discharge), or EKG if your healthcare provider thinks they are medically necessary.
How can I get the 15% discount?
Self-pay patients can receive 15% off by providing the front desk staff with the discount code. This discount does not apply to vaccine costs and lab tests. The discount code is found in the About Us page of our website.
I have an in-network health insurance, but I want to pay in full with cash after the visit. Can I qualify for the self-pay program?
No, you do not qualify. We must send a claim to your insurance company.
I have health insurance, but I have a high deductible plan. Can I choose to pay as a self-pay patient?
No, sorry but you cannot. This program is only for patients without health insurance.
I have health insurance, but City Care Family Practice is not in-network with my insurance. Do I qualify?
Unfortunately, no. After paying for the visit, we can print out a claim that you can send to your insurance company for reimbursement.
I want to do blood tests for cholesterol and blood sugar. Is this part of the general office visit fee?
No, blood tests such as for cholesterol and blood sugar are not part of the general office visit fee. We can draw blood at City Care Family Practice and send your specimen to the laboratory. You will be billed separately by the laboratory which offers 70% discount for uninsured patients.
When is the payment due?
Payment is due at the time of service. We accept cash or credits cards. We do not accept checks or money order. We do not offer payment plans
